This elevated estrogen, even before a true LH surge, can cause OPKs to register a false positive. The increase in estrogen isn’t always perfectly timed with the actual ovulation event.

Consequently, a positive OPK result on Clomid doesn’t always guarantee imminent ovulation. You might experience a positive OPK several days before, or even after, your actual ovulation. Therefore, relying solely on OPKs while on Clomid for timing intercourse can be unreliable.

Tracking basal body temperature (BBT) alongside OPK testing offers a more complete picture. A sustained temperature rise after a positive OPK confirms ovulation. Ultrasound monitoring by your fertility doctor provides the most accurate confirmation of follicle growth and ovulation.
